ROSEVILLE — Through Honoring Choices Minnesota, an Advance Care Planning helpline is open for appointments through Sept. 30.
"The goal of ACP is to help others understand what health care choices you would make if you could not communicate," a release said.
The helpline will offer step-by-step guidance for Advance Care Planning, as well as Health Care Directive assistance.
"Knowing your voice is heard when making decisions about health care is important," the release said. "Advance Care Planning is the process of preparing for a time when you may not be able to make your own medical decisions. The best time to make these decisions is when you are able to make your own choices."
The helpline is free to call, and appointments are available from 9 a.m. to 7:30 p.m., Monday through Friday.
